Triangular yellow plastic tag, printed in green on front, "ROCHESTER COUNTRY CLUB / JACK GALE / GOLF PROFESSIONAL", with image of golfers above and PGA logo below; applied blue plastic label with white embossed lettering, "KAY ALLEN". Reverse printed in green, "Rochester Country Club / ROCHESTER, NEW HAMPSHIRE", with image of golfers in center. Adjustable white plastic strap attached through slot at top.
