Obverse: cast profile of Old Man of the Mountain, with raised lettering around perimeter, "STATE OF NEW HAMPSHIRE / OPERATION DESERT STORM." Reverse: raised lettering, "For Service In / OPERATION / DESERT STORM / Thank You / from the people of New Hampshire / Presented by / Governor Judd Gregg"; New Hampshire State Seal in center. In square cardboard box with removable cover and velvet-covered foam interior; printed paper label adhered to bottom exterior: "hampshire / pewter / WOLFEBORO, N.H."
