Monogram "CSF". "C" is slightly sculpted left edge with some diagonal shadow marks as do other characters. "S" has some vertical shading in it and the "F" has light horizontal shading. This is on the backside of a printed form. Some portions of it read: "or Mdse" "S. or W." "Other" "Tons".
Writing on back in pencil: "Clapp 1/28/58". - Material:
