- Monogram Pattern, Pencil. Monogram reads in left center of sheet in pencil: "GAR". All letters have short diagonal shadow lines outside their edges. "G" has internal shading with mostly horizontal strokes. "A" "R" have internal shading with variety of stroke directions.
On front lower left corner: "Clapp 1/28/58".
Monogram on back of printed form. Portions of form "Train No." "From" "To" "Conductor" "Snowfall Inches". "Seals or Mdse." Below that "N. or E." "S. or W." "Other" "Tons" "Home Route or Destination". - Material:
