On front writing in pencil: "Clapp 1/28/58". Letters on front of monogram: "GAT" in pencil. Some shadows behind the letters. The "G" has vertical pencil lines in it. The "A" has a few horizontal pencil lines in it. "T" has diagonal pencil lines.
Paper is printed form with the title caps: "MATERIAL MOVE ORDER". Other parts of the form are "Date" "S.O.No." "Rail or Trailer No." "From" "To" "Description". "Time Requested" "A.M." "Handle by Tractor No." "Time" "Requested by" "Rec'd by". Next one are form numbers upper right corner: "BS-14 6-28-100M".
On back there is writing and printed materials. Writing is on right side facing to center in pencil: "M.D.INC." - Material:
