- Hat Form. Wooden crown form carved from a solid block of wood. Interior of base has been hollowed out for weight reduction. Central spindle with 0.75" hold in center. Stamped on base, "5 7 5/8 902 / SAMUEL LEBOW CO." Ridge on upper side running the length of the form.
